Health assessment check list for employees

Before your health assessment

Have I…

  • been directed in writing to undergo a health assessment?
  • familiarised myself with my agency's health assessment policy?
  • familiarised myself with my rights of review?
  • reviewed my leave balances and submitted all medical certificates and any outstanding leave applications for approval?
  • been assigned a case manager or been given a contact in the human resources team?
  • discussed any concerns with my manager or human resources team (or case manager)? Do I have all the information I need? Have all my questions been addressed?
  • informed my manager or my human resources team if I have submitted a workers' compensation claim? Have I told my manager or my human resources team if I have attended, or will be attending, any Comcare medical assessments?
  • received the details of my health assessment appointment? Do I know where to go and who I am seeing?
  • been advised if my appointment will count as work time and if my travel expenses will be covered? Do I know who pays for the health assessments? Do I know who covers the cost of a late cancellation of a health assessment?
  • advised my manager or human resources team if I am unable to attend at the nominated time? Have I rescheduled my appointment?
  • advised my manager or human resources team if I need an interpreter or reasonable adjustments for my health assessment?
  • arranged for a support person to be present at my health assessment, if I would like one? Have I told my support person that I want them there to support me, but that they cannot participate in or comment on the health assessment? Have I told my manager or my humans resources team that I would like to have a support person at my health assessment?
  • collected or prepared any relevant information for my health assessment (such as supporting information on my health condition or my duty statement)?
  • accessed support through the Employee Assistance Program?

After your health assessment

Have I…

  • received a copy of the report from my health assessment?
  • discussed the report and its recommendations with my manager or human resources contact (or case manager)? Do I have all the information I need? Have all my questions been addressed? Do I know what I can do if I disagree with the report?
  • raised any concerns about the process with my human resources area?
  • understood and agreed to the next steps?
  • accessed support through the Employee Assistance Program?
